2008-2009 INFLUENZA VIRUS VACCINE UPDATE

Posted: 08/29/2008

The Supply Service Center intends to start shipping the flu vaccine the week of September 8th. In shipping the orders, the Center will maintain a first-in, first-out policy. As always, we will do our best to meet our customer’s needs, whenever possible.

In order to facilitate a smooth distribution process, it is paramount that we are made aware of any address changes, access restrictions, and hours of operation changes. Please notify the Center immediately if any of these conditions apply.

Due to an anticipated surplus of flu vaccine, we are able to accommodate additional vaccine requests and related medical supplies, including syringes, needles, and alcohol pads. This year, the vaccine offered is FluLaval ®, and is for patients 18 years and older. Our selling price is $75.66 for a ten-dose vial. Please note that when ordering, NSN 6505-00-000-7471 is to be utilized.. You can fax your request to (410) 642-6249/2616 or contact Customer Service at 1-800-642-0263 or 410-642-2244.

We will make every effort to help ensure your vaccine inoculation programs run as effortless and efficient as possible. Finally, please remember that flu season is from September to February, with February often considered the peak of influenza activity.

If you have any questions or concerns, please contact Annette Quinones at 1-800-642-0263 Ext: 1386.

URGENT PRODUCT RECALL

Posted: 08/05/2008

Dear Valued Customers:

Akorn’s Cyanide Antidote Kit (6505-01-143-4641) incorporates a BD 60 ml syringe as one of several components of this kit. BD has determined that unit package seal integrity (and resulting product sterility) of this 60 ml syringe can be adversely affected when exposed to low atmospheric pressure experienced at high altitudes (e.g. during product shipping). All other components in the kit remain unaffected by this recall, and are considerate to be acceptable for use.
ALL BD lot numbers beginning with 5, 6, and 7 are affected, as well as those lot numbers beginning with 8. Respective lot numbers are imprinted at one end of each syringe pouch. An attached list of the affected lots will be attached.

We ask for your cooperation in taking the following action:

• Please examine your inventory at once. If you have the recalled product in question, immediately cease distribution.
• Remove ONLY the syringe from the kit to be destroyed. Remain components acceptable for use.
• We strongly recommend that the antidote kit remain in your possession and the 60ml syringe be destroyed.
• Please fill out the attached Akorn’s replacement form to obtain a 60 ml syringe replacement for your kit at no charge.
• If you have any questions please call Akorn Inc Customer Service at 1-800-932-5676 (menu option #1)

If you have any questions about this recall, please contact Quality Assurance at 1-800-642-0263. We apologize for any inconvenience this recall presents, and thank you in advance for your prompt attention to this urgent request.
